How to Complete the Recertification Journal/Application or Recertification Application only
Download the Recertification Journal and Application (PDF, 332 KB) or Recertification Application only (PDF, 40 KB).
You can type directly in the documents above. As you are typing, tab or click on the specific areas where you want to enter your information. You will need to scroll down to reach the other areas of the recertification journal. Do not type more than the allotted lines for each category. As you enter your information, each column of the recertification unit credit will auto-tally for you. You will not be able to save your entered category items. Be sure to print out two copies—one for submitting and one for your records.
